Optional removal of OPI references of replaced images

Info

Added new preference RemoveOPI with type bool and with
default FALSE. Preference RemoveOPI determines whether
pdfresolve removes OPI references of replaced images or not.
Also fixed:
- now pdfresolve -v prints the file names of inserted images
- pdfresolve exited with an error message if it encountered
/ID entries in OPI 1.3 dictionaries which are not a valid
Macintosh file ID
